Client Intake Sheet & Pain ChartOn your first visit the Intake Sheet and the pain Chart will need to be filled out. You can email them to me in advance so I can familiarize myself with your situation before you arrive. This will not change the initial visit fee, but we will have more time to talk things through in depth.
